The enrolment form

The form is the centre's own printed form, kept in its original PDF format — the same file the original site offered.

The form opens in any modern browser — Adobe Reader is no longer needed to view or print a PDF, so the old “Get Acrobat Reader” badge has been retired from this page.

How to book a place

1. Download and print the enrolment form.
2. Fill it in and bring it to your first class, or email it to info@listenuppup.com.
3. Call 405-224-7715 first so we can tell you which class starts next and whether there is space in it. Class size is limited by the training hall.

Fees and concessions

There is no charge for Pets & People dogs, or for Assistance Dogs in training. For every other class, the current fee is confirmed by telephone when you book.

What to bring to your first class

Bring your dog on a collar and lead, a mat or towel for it to settle on, and small soft treats the dog enjoys. Bring the whole family if you can — everyone who lives with the dog should give it the same instructions.

If your dog is unwell, or has been vomiting or has diarrhoea in the previous 48 hours, leave it at home and come to the next session without it.
